The secure SwyftOps Family Portal gives Grace Givers clients and their authorized contacts convenient access to schedules, care information, documents, invoices, and communication with our administrative team.

The Family Portal Is Our Standard Method for Secure Communication

Clients and authorized family members can use the Family Portal to:

  • View the current care schedule
  • Request schedule changes or additional shifts
  • Send non-urgent questions and updates
  • Review care notes and daily care summaries
  • Access invoices
  • Submit concerns or complaints
  • Communicate securely with Grace Givers administrators
  • Review and sign documents through CareVault

Using the portal helps prevent missed messages, protects private information, and creates a secure record of requests and communication.

Our office remains available by phone for clients who need assistance. However, routine schedule information and non-urgent requests should be accessed or submitted through the portal whenever possible.

View Your Weekly Schedule

Schedules are published each Thursday by 10:00 p.m. for the upcoming Saturday-through-Friday schedule period.

Clients and authorized family members can access the portal to review:

  • Scheduled dates and shift times
  • Confirmed caregiver assignments when available
  • Schedule updates and changes
  • Additional confirmed visits

Schedules and caregiver assignments may occasionally change because of client needs, caregiver availability, illness, emergencies, or other scheduling circumstances. The portal will reflect the most current confirmed information available.

Clients who are unable to access the portal may call the Grace Givers office for schedule information.

Request Schedule Changes or Additional Care

Clients and authorized family members may use the portal to:

  • Request a change to an existing shift
  • Request an additional shift
  • Ask about increasing or decreasing scheduled care
  • Notify Grace Givers about appointments or schedule conflicts

Submitting a request through the portal does not automatically change the schedule or guarantee caregiver availability. A Grace Givers team member will review the request and confirm whether it can be accommodated.

Review Care Notes and Daily Care Summaries

Authorized users may review information documented about completed visits.

Depending on the visit and care plan, this may include:

  • Previous caregiver shift notes
  • Daily care summaries
  • Completed care activities
  • Observations related to the client’s routines and support
  • Other information documented as part of the client’s care record

Care notes reflect what occurred during a particular shift. If you have a question or concern about documentation, please contact the Grace Givers office rather than contacting the caregiver directly to request changes.


View Invoices

Clients and authorized billing contacts may use the portal to review available invoices and billing information.

Submit Questions, Concerns, or Complaints

Clients and authorized family members may submit questions, concerns, or complaints through the Family Portal.

Grace Givers takes client concerns seriously. Providing specific information—including the date, shift, individuals involved, and nature of the concern—helps our care-management team review and respond appropriately.

The portal should not be used for emergencies or concerns requiring an immediate response.

For emergencies, call 911 when appropriate. For urgent Grace Givers matters, call 931-337-0890 and leave a message for the on-call supervisor.


Secure Two-Way Messaging

The Family Portal provides HIPAA-compliant two-way messaging with Grace Givers administrators.

Secure messaging may be used to:

  • Ask non-urgent questions about care
  • Discuss scheduling needs
  • Request clarification about documentation
  • Share updates relevant to the client’s care
  • Ask billing or portal-related questions

Please do not rely on portal messaging for emergencies, immediate call-outs, or time-sensitive same-day concerns. Call the Grace Givers office when prompt assistance is needed.


Access CareVault

CareVault provides secure access to documents related to the client’s services. Depending on the client’s care and enrollment status, authorized users may be able to:

  • Review and electronically sign documents
  • Access completed care plans
  • Review completed service agreements
  • View forms and acknowledgments
  • Access other documents pertinent to the client’s care

Getting Access to the Family Portal

Each authorized client or family member must have a separate portal account connected to an individual email address.

1. Grace Givers creates your access.

Our office enters the authorized user’s name and email address.

2. Look for your email invitation.

SwyftOps will send an invitation with instructions for creating your password. Watch for your invitation to possibly come through your SPAM or Promotions Folder.

3. Log in securely.

Visit https://swyftops.family and enter your individual email address and password.

Please do not share your login information. If another family member needs access, contact Grace Givers so an individual account can be created with the appropriate permissions.

Notify our office when someone’s authorization should be changed or removed.

Need Help?

If you have not received your invitation, cannot log in, or have questions about the portal, please contact Grace Givers Home Care. Once you have created your password, you may reset it at any time by selecting “forgotten password” on the login page of the family portal. Password resets must be initiated by the user by using the email address on file with Grace Givers.
